The harmony is deliberately minimal. Patrick Leonard and Madonna stripped the rock-demo complexity from the original and replaced it with a hypnotic repeating loop. The progression barely moves; the arrangement does the work instead, adding and subtracting synth layers, percussion, and vocal stacks. That restraint is a production choice, not a limitation — it keeps the rhythmic drive unbroken for the dance floor.

Songwriters Gardner Cole and Peter Rafelson spent close to a year developing what they called 'Follow Your Heart', a mid-tempo rock song they had written with Cyndi Lauper in mind — though she never heard the finished demo. Cole felt the track 'didn't really hit us as a hit song' but they kept refining it anyway. Cole's manager Benett Freed was also working with Madonna's team, who were assembling material for True Blue, and offered them the song among a handful of others. Madonna's team selected it, which surprised Cole given that it was 'more rock and roll than I thought she would want to go'. At Madonna's request, Cole recorded a fresh demo with his girlfriend Donna De Lory on lead vocals; Madonna liked it so much she later hired De Lory as a regular backup singer. Working with Patrick Leonard, Madonna added a bassline, shifted the track from rock to dance-pop, rewrote sections of the lyrics — earning a co-writing credit — and renamed it 'Open Your Heart'.

Originally titled 'Follow Your Heart' and conceived for Cyndi Lauper, the song was overhauled by Madonna and producer Patrick Leonard into a driving synth-pop single. Released as the fourth cut from True Blue in November 1986, it reached number one on the US Billboard Hot 100 and gave Madonna the distinction of being the second female artist — after Whitney Houston — to score three chart-toppers from a single album. Jean-Baptiste Mondino's peep-show music video made it one of the decade's most debated clips.
